When filling out the second page of the form, keep the following in mind:

• Please enter your lodge name and number in the entry marked 'Company'.
• Enter your address and other information in succeeding lines. This is your address, NOT the address of your lodge!
• If the donation is a memorial, enter the person's name under 'Shipping Information'. If possible, provide an address or other contact information for the memorial.
